A transcutaneous oxygen monitor (TcO2 monitor) is a device used to measures the amount of oxygen in the blood through the skin. It works by applying a small sensor to the skin and heating it to a temperature of about 42 degrees Celsius. This allows the dilation of the blood this provides more blood to the sensor. The sensor measures the amount of oxygen in the blood and sends the reading to a display device.

It is a non-invasive, clinically-approved method to obtain skin oxygen levels. The method is quantitative and measures oxygen delivery to the skin from underlying tissue. It has been used to monitor transcutaneous oxygen tension (TcpO2) in the skin, especially for premature infants, but also for adults in the intensive care setting. Physicians also use TcO2 monitor to determine the adequate blood flow exists to heal lower extremity wounds, such as venous stasis or diabetic foot ulcers. Vascular surgeons use TcO2 measurements to assess the need for amputation and determine the level where the amputation should take place.

The TcO2 method has some limitations This method is prone to calibration errors and the electrode consumes oxygen during measurement and is thus not capable of measuring low levels of oxygen that may be present in the skin under room temperature. To compensate for this, TcO2 uses a heated metal electrode placed directly on the skin. During measurement, the skin beneath the electrode is heated up to 44°C, which dilates the vessels in the underlying tissue and increases oxygen permeability.

A wound healing monitor is mainly used to monitor the oxygen levels in wounds which may help in monitoring the progress of the wound healing. The Transcutaneous Oxygen Monitor (TcO2) provides a non-invasive method of measuring the oxygen tension in the tissue beneath a wound dressing. The TcO2 sensor is attached to the skin near the wound site. The most common location is over the wound bed, adjacent to the wound margins or on the healthy skin around the wound. The Transcutaneous Oxygen Monitor sensor consists of an oxygen-permeable membrane that is placed against the skin. Before taking the readings, the machine is calibrated which gives accurate readings. The sensor present in it measures the oxygen tension in the tissue by monitoring the diffusion through skin. The sensor detects the partial pressure of oxygen (PO2) in the tissue and displays the values on the monitor. If the tissue oxygen level is low it indicates poor healing process and if the oxygen level is adequate the tissue the wound id said to be healing.

  • Blood Gas Monitoring

The blood gas monitors avoid the need of taking blood out using a syringe and reducing the pain caused due to it. They provide real-time measurements, which allow the doctors to observe any fluctuations or changes in the patient's oxygen status. The sensor is placed on the patient's skin, usually on the earlobe, forehead, or sternum. The sensor contains a heating element and an electrode that measures the pO2. If the sensor is properly placed, it can start the measurement process. The heating element warms up the skin, which increases blood flow to the area and enhances oxygen diffusion through the skin. A calibration gas mixture is used to calibrate the machine to give measurements more accurately.

In the case of diabetes-related complications like peripheral arterial disease or foot ulcers, TcO2 can help to evaluate the adequacy of blood flow to the affected limb or wound site. Low oxygen levels in the tissue indicates poor perfusion causing a need for evaluation and intervention. Transcutaneous oxygen monitoring can help the doctors to make the treatment decisions. If a diabetic foot ulcer is present and the TcO2 indicates low tissue oxygenation, interventions such as revascularization procedures (e.g., angioplasty or bypass surgery) or wound care treatment may be considered to improve blood flow and enhance tissue healing.
